4. here in wash state we just need proof ( med records) that we discussed illness..

...with our MD within the last year. It just has to be mentioned in the dr notes..we take that to a state approved 'provider' who makes sure our paper work is in order (the provider i see is a Naturopath)..and that our condition meets state guidelines and from then we can choose go to the local med approved pot shop to get registered with the state ( just gives us access to more grows and more oz's)..this is optional...but then we get a cute card with a horrid photo. Or just go buy pot.

But if I were in the position you are now, I would call.
