8. NASA never had problems like this with the Saturn 5 lunar rockets.

The closest they came was combustion instability in the engines and they fixed that by installing baffles on the fuel injection plates to disrupt the oscillating pressure waves.

13 Saturn 5 launches during the Apollo program and no losses.
