Leqembi (lecanemab) is an FDA-approved IV treatment for early Alzheimer’s disease that modestly slows cognitive decline, but does not cure or reverse the disease.

Once treatment starts, doctors closely monitor patients through bi-weekly infusions, scheduled brain MRIs, and continuous symptom tracking, especially during the first 3 months when risks are highest.

MRI surveillance focuses on detecting ARIA—brain swelling or small bleeds—which can require pausing or stopping treatment.

Cognitive testing tracks the rate of decline, not improvement. Most critical safety signals come from caregiver observations between visits.

In practice, Leqembi is less about the infusion itself and more about ongoing imaging, vigilance, and risk management.
